What a help desk approval evidence record should contain
Give a decision owner enough context to approve sensitive support work without hunting through several messages.
Direct answer
An approval request should ask for one decision. Put the requested action, affected account or service, reason, impact, deadline, and supporting evidence near the top of the ticket.
Separate verified facts from the requestor’s explanation. If a detail is still unknown, label it as unknown and name the person who can confirm it.
The support specialist may prepare the request and complete the approved follow-up. The decision owner remains responsible for policy, identity, money, security, or account ownership choices.
Close the loop in the same record. Capture the decision, approver, time, conditions, and customer update so a later reviewer can understand why the action happened.
