A portal form should collect the smallest useful set of facts for its request type. Start with the system, impact, attempted step, and requested outcome, then add fields only when they change the route.
Use plain customer language beside internal labels. Explain why a field matters and tell the requester what never belongs in the form, such as passwords or recovery codes.
Give different paths to different risk levels. A general question should not expose the same fields or ownership route as an account change.
Compare incomplete submissions with the questions specialists ask after intake. Remove fields that do not help and add a prompt only when it prevents repeat contact or unsafe handling.
